I get these too, sometimes over copper wire and over surface imperfections that form an air bubble. I always assumed they were just "dry spots" where the epoxy did not saturate the cloth all the way through. I grind em away like you did though I am too lazy to pick out the dust, so I feather the crater edges down.

I never thought of your explanation of contamination. It seems plausible.

If your theory is wrong about contamination, and it is rather the air bubble that causes the problem, then countersinking is ok and all you need to do is patch the depression flush with the surface, with thickened epoxy. No more air bubble and it will fill-coat properly.
