I use a T-Rac (small truck version of Trac-Rac). Much more solid and stable than racks on the cab. It gives a wide enough spacing (similar to Kurt's camper shell approach) that I don't need bow or stern lines. It'll also support much more weight (700 + lbs) than you'd ever wantthat high up on a pickup. Mileage with the racks installed is 30 mpg for a Chevy S10.
