I second Kurts recommendation to read "Deep Trouble" by Matt Broze and George Gronseth. It is a compilation of stories from Sea Kayaker magazine. It is very readable, with good descriptions of situations encountered and analyses of what happened and should have happened. The bottom line is to develop skills through classes and clinics, think about what might go wrong, and be prepared with the required skills and equipment for the situations we put ourselves in. Kayaking is not just a physical activity, it is also a mental activity.
